Comprehensive Waiver and Treatment Agreement

Welcome to The Toronto Light Therapy Clinic. Please read the following agreement carefully before proceeding with our services. By digitally signing this document, you acknowledge that you have voluntarily enrolled in services provided by The Toronto Light Therapy Clinic, which includes but is not limited to electromagnetic body shaping services, red light therapy, and infrared light therapy (hereinafter referred to as the "Services"). In consideration of being permitted to participate in these Services, you agree to the following terms and conditions:

1. Assumption of Risk:

I understand and acknowledge that the Services provided by The Toronto Light Therapy Clinic involve inherent risks, including but not limited to the risk of injury, illness, or adverse reaction. I voluntarily assume all risks associated with participating in these Services.

2. Release of Liability:

I hereby release, waive, discharge, and covenant not to sue The Toronto Light Therapy Clinic, its owners, employees, agents, and affiliates (hereinafter referred to as the "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including but not limited to personal injury, illness, or property damage, that may be sustained by me while participating in the Services, whether caused by the negligence of the Released Parties or otherwise.

3. Indemnification:

I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all liabilities, damages, losses, costs, expenses, and attorney fees incurred by the Released Parties arising out of or related to my participation in the Services.

4. Medical Clearance:

I acknowledge that it is my responsibility to consult with a qualified healthcare professional before participating in the Services, especially if I have any pre-existing medical conditions or concerns. I agree to disclose any such conditions to The Toronto Light Therapy Clinic prior to participating in the Services.

5. Compliance with Instructions:

I agree to comply with all instructions provided by The Toronto Light Therapy Clinic regarding the use of equipment, devices, or products during the provision of Services. I understand that failure to follow these instructions may increase the risk of injury or adverse effects, and I assume full responsibility for any consequences that result from non-compliance.

6. Treatment Agreement:

I understand that the Services provided by The Toronto Light Therapy Clinic are for therapeutic and cosmetic purposes and are not a substitute for medical treatment or advice. I consent to receive these Services and acknowledge that results may vary depending on individual factors such as age, genetics, and lifestyle.

7. Informed Consent:

I acknowledge that I have been fully informed of the nature, purpose, risks, and benefits of the Services provided by The Toronto Light Therapy Clinic. I understand that no guarantees have been made regarding the outcome of these Services, and I agree to assume any risks associated with their use.

8. Photographic Release:

I grant The Toronto Light Therapy Clinic the irrevocable right and permission to use photographs or videos taken of me during my participation in the Services for promotional and marketing purposes without compensation to me.

9. Confidentiality:

I understand that any information disclosed to The Toronto Light Therapy Clinic during the provision of Services will be kept confidential, except as required by law or with my express consent.

10. Miscellaneous:

This comprehensive waiver and treatment agreement shall be binding upon my heirs, executors, administrators, and assigns. If any provision of this agreement is found to be unenforceable or invalid, such provision shall be severed from the remainder of this agreement, which shall continue in full force and effect.
